You can do either. Placing it directly on the ground is perfectly fine, though many hunters prefer using a stump or a raised platform to keep the block out of the dirt and mud, which helps it last longer.

Longevity depends heavily on the size of your local deer population and how frequently they visit the site. In areas with high deer density, you can expect the block to be consumed more quickly, while lower-traffic areas may see the block last several weeks.
